1. Download both entry form pages below and fill out ONE Entry Form per horse.
    
2. Please send a copy of the following:
     -- Registration Papers
     -- Current Negative Coggins
     -- Current APHA Cards ie: Youth, Novice Youth, Novice Amateur and Amateur
    
3. If you have not been assigned a back number from either the NMPHC or MPHC please leave the back number blank and a back number will be assigned to you.
    
4. Upon arrival please check in with the show office to pick up your show packet and review your entries. You will need to leave an open check for your show tab.
    
5. You are not required to send payment with your entry.
    
6. Please make sure your entry form is filled out completely regardless if this horse has been shown at a NMPHC or MPHC show.
    
7. We encourage you to pre-enter. Once the horse is in the system, the office staff will be able to add or scratch classes quickly and efficiently.
    
8. Please have your pre-entries post marked by October 19th to be entered in a drawing for a free stall at the Fall Color Classic.
